Training Agreement

1.  Do you or your child suffer from any medical conditions that might affect your ability to perform physical exercise? Conditions might include:

• head injury
• seizures
• diabetes
• muscular / skeletal conditions • asthma
• infectious diseases

If so, you should consult your doctor prior to commencement of training. Your instructor should also be made aware of any conditions that might require assistance during your training. You can discuss your situation with them or write additional notes on this page.

2.  Release and Waiver of Liability

agree to participate in training and receive instruction in the art of Kodokan Judo and/or Brazilian Jiu-Jitsu at Pacific Grappler Dojo (PG Dojo). In consideration of the service provided to me by the PG Dojo instructors, I hereby freely and knowingly accept and agree to the following:

Assumption of Risk.

I understand that Judo and Jiu-Jitsu is a potentially hazardous activity that involves inherent risks of serious physical injury. With full knowledge of the risks involved in Judo and Jiu-Jitsu, I expressly assume all the risks of harm to myself arising from the practice of Judo and Jiu-Jitsu at PG Dojo.

Release of Claims and Waiver of Liability.

I hereby expressly, and for all time, on behalf of myself and personal representatives, release and agree to hold harmless, PG Dojo from any claim, demand, or cause of action at law or equity from any injury to me that arises, or might have arisen, from my participation in the practice of Judo and  Jiu-Jitsu, or from my use of any of the equipment of PG Dojo.

Terms and Conditions.

I have read, understand, and accept, the terms and conditions of joining PG Dojo, as outlined in this document.

Photos permission.

I hereby grant permission for PG Dojo to use photos or videos of me and/or my child for legal custody in publications, online publications, websites, social media and presentations, for the purposes of promoting and advertising the gym and relative programs.
